Has a claim been rejected? We apologize for the inconvenience. Claims are rejected because a member's drugs are either non-formulary or are subject to a Formulary Restriction. If you experience a rejected claim, follow these steps:

  • Review your patient's formulary and find Prior Authorization and Exceptions forms.
