Charles E. Hallberg, JD
President and CEO
Charles E. Hallberg founded MemberHealth, Inc. (MHRx) in 1998, bringing over 25 years of pharmaceutical industry leadership to the Company. Mr. Hallberg's experience has allowed him to grow MemberHealth into a leading Prescription Benefit Management (PBM) company, and the nation’s fourth largest Medicare Part D Prescription Drug Plan.
His overall business philosophy of transparency sets MemberHealth apart from the competition, offering innovative products with consumer driven formularies, designed to provide members with the most cost effective, therapeutically appropriate, prescription drug care.
These innovative products are needed, as demonstrated by MemberHealth’s winning of the #10 slot on the 2006 Inc 500 list of fastest growing companies.
As vice president of marketing and sales and a principal for Continental Managed Pharmacy, Mr. Hallberg was responsible for the administration of an integrated prescription benefit mail service pharmacy, product design, implementation procedures, and sales management. He also founded Automated Scripts Net, a national PBM company.
Mr. Hallberg served as executive vice president, administration, general counsel and secretary at General Computer Corporation (now MedE AMERICA CORPORATION; acquired by WebMD). Previously he served as Corporate Counsel for Revco D.S., Inc. where his responsibilities included general corporate law, employee benefits, merger and acquisitions, FDA manufacturing compliance and intellectual property law.
A Finalist in the 2006 and 2007 Ernst & Young Entrepreneur of the Year competition, Mr. Hallberg remains focused on continued growth of the company.
Mr. Hallberg earned his bachelor of arts degree from the University of Massachusetts and his juris doctorate degree from Case Western Reserve University. He is a member of the State of Ohio and Federal bars.
David S. Azzolina
Chief Financial Officer
As chief financial officer, David Azzolina is responsible for MemberHealth's financial matters, including financial strategy, compliance, reporting and financial support of operations.
Previously, Mr. Azzolina served as corporate treasurer for Century Business Services, Inc. (CBIZ). Prior to joining CBIZ, he spent 13 years at NutriScience Technologies, Inc. (formerly Bioproducts, Inc.), managing a broad range of financial assignments, including strategic initiatives, financial planning and analysis, accounting, and cash management.
Mr. Azzolina's educational background includes a bachelor of science degree in accounting from The Ohio State University and a master's degree in business administration from the University of Akron. He is also a licensed certified public accountant in the State of Ohio.
Jane Koehl-Colling, Esq.
General Counsel & Director of HR
Ms. Koehl-Colling serves as MemberHealth's General Counsel and Corporate Secretary as well as heads the company's Human Resources department. Ms. Koehl-Colling has over 15 years of experience in law, compliance and human resources administration.
Prior to joining MemberHealth, Ms. Koehl-Colling was with Squire, Sanders & Dempsey, LLP from 2000-2005. Prior to joining Squire Sanders, Ms. Koehl-Colling was in private practice in the Cleveland, Ohio area specializing in the representation of corporate clients in complex domestic relations litigation matters.
Ms. Koehl-Colling holds a bachelor of arts degree in political science from the University of Wisconsin and a juris doctorate from the Cleveland-Marshall College of Law. She is a member of the State of Ohio bar, the American Health Lawyers Association, the Association of Corporate Counsel and the Society of Corporate Secretaries & Governance Professionals.
Samuel Rajan, R.Ph.
Vice President, Clinical Operations
An accomplished corporate strategist and clinician, Mr. Rajan is responsible for clinical program development.
With nearly 15 years of leadership experience building clinical pharmacy and pharmacy benefit management teams at top-tier companies, such as Kaiser Permanente, HealthNet and Caremark, Mr. Rajan's exceptional track record is based on his philosophy of improving patient care first. He is known for his ability to quickly identify and diagnose clinical impediments as well as supply channel synergies, and his ability to pull these two different areas together is his greatest strength.
Mr. Rajan earned a bachelor of science degree in pharmacy from the University of Toledo. He also studied health economics at the Wharton School of the University of Pennsylvania and has presented in managed care forums, including the Generic Pharmaceutical Association and Pinsonault Associates PBM Symposium. Mr. Rajan also serves on the Leadership Board of Director for the American Diabetes Association of North East Ohio.
D. Alan Scantland
Senior Vice President for Corporate Development
D. Alan Scantland is responsible for assuring and enhancing the growth capacity of MemberHealth. As a senior leader with 30 years experience, he maintains an outstanding track record in business development and technology management with market-leading firms.
As former head of corporate development of Into Great Companies, a venture capital firm, he identified and evaluated attractive growth investments. Hired by Express-Med to build growth engines for the company, his successes included new business development, alliance building, technology management, and executive coaching.
Through his leadership at Battelle, a global science and technology enterprise that develops and commercializes technology and manages laboratories, Scantland built profitable business relationships with the company's top ten commercial customers in the U.S., Asia, and Europe.
Mr. Scantland earned a master of science degree in agricultural economics from The Ohio State University and a bachelor of arts degree in biology from Earlham College.
Len Ploskonka
Chief Information Officer
Len Ploskonka serves as MemberHealth's chief information officer. He is responsible for the development, execution and oversight of information technology, strategic planning, and other functions essential to the Company's ability to deliver superior service and products to consumers, clients and health care professionals.
Mr. Ploskonka's professional background includes more than 30 years of experience in all phases of healthcare information technology, including software development, strategic planning, application design, account management and technology integration.
Prior to joining MemberHealth, Mr. Ploskonka served as vice president of information technology at Catalyst Rx (aka HealthExtras), a prescription benefits manager (PBM); president/CEO of Intellect Inc., an information technology consulting firm; senior vice president and CIO of information systems at SysteMed (PBM); and vice president of information systems at Insurx (PBM).
John G. Kloss
Deputy Director for the Community CCRx Program
As vice president of Medicare programs, Mr. Kloss is responsible for the beneficiary services of Community CCRx, a Medicare-approved prescription drug plan MemberHealth administers. Prior to assuming this role, Mr. Kloss was responsible for financial matters associated with federal programs administered by the Company.
Mr. Kloss previously worked at WebMd (formerly MedE America), a provider of transaction processing services to MemberHealth. In 1998, he served as controller of MedE America's Pharmacy Division. After MedE America was acquired by WebMD, Mr. Kloss was promoted to vice president and general manager of PBM Services. Mr. Kloss also held positions in operations, IT and finance at Rockwell Software, Rockwell Automation, and Reliance Electric.
Mr. Kloss' education includes a bachelor of science degree from Bowling Green State University and master's degree in business administration from Baldwin-Wallace College.

Marc Bralts is the vice president of business development at MemberHealth, where he is responsible for management of the pharmacy marketing activities and network operations. He also collaborates with the sales and marketing teams on strategy and lends expertise in product development. Mr. Bralts brings with him more than 10 years of experience in the PBM industry. Prior to joining MemberHealth in 2006, Mr. Bralts acted as a consultant. He also spent eight years at ScriptSave as the vice president of network and corporate accounts.
Mr. Bralts earned his bachelor’s degree in communication and political science from the University of Arizona and is a Certified Pharmacy Technician. He has also been directly involved in the administration of the Medicare Modernization Act (MMA) since its inception in 2003.
Ann Vickers
Director, Marketing
Ann Vickers is the director of marketing at MemberHealth, where she is responsible for all internal and external communications as well as the development and execution of strategic marketing initiatives.
Through her more than 20 years of marketing and communications experience, she has faced a variety of communication opportunities and challenges in competitive settings. Her experience enables her to align the strategic needs of the organization with the ever-changing demands and needs of both the health industry and consumers.
Prior to joining MemberHealth in 2006, Ms. Vickers was the director of communications for Delphi Packard Electric, where she held various communications positions in her more than 13 years of service. She also worked for General Motors for seven years.
Ms. Vickers earned her bachelor’s degree in journalism from the University of Wisconsin, Madison, where she graduated with distinction.
Jim Jacko
Vice President, Operations
Jim Jacko is the vice president of operations at MemberHealth. Using his more than 22 years of experience, he is responsible for enrollment, PDE claims processing, fulfillment and member services as well as managing day-to-day relations with Computer Science Corporation (CSC).
Prior to his joining the company in 2007, he spent eight years as the director of finance with PSC metals, the third-largest scrap recycling company in the United States. Before PSC, he was also a corporate controller with BridgeStreet Accommodations and held several ranking finance-related positions at Capital American Financial Corporation.
Mr. Jacko earned his bachelor’s degree in business administration from Wittenberg University. He is also a Certified Public Accountant in the State of Ohio.
